Southern Roots Endodontics
Website Terms of Use
Effective date: 10/6/2026
Welcome to southernrootsendo.com (the “Website”), operated by Southern Roots Endodontics (“we,” “us,” or “our”). These Terms govern your use of the Website and its related online features. By using the Website, you agree to these Terms. If you do not agree, please discontinue use.
Informational purposes and dental disclaimer
Website content—including text, photographs, graphics, videos, and patient education materials—is for general information about endodontic care, including root canal treatment, retreatment, and endodontic surgery. It is not individualized dental or medical advice, diagnosis, or treatment. Treatment options, suitability, risks, and outcomes depend on your clinical circumstances and examination.
Consult a qualified dentist or healthcare provider about your specific concerns. Do not disregard professional advice or delay treatment because of information on this Website. We make reasonable efforts to keep content accurate but do not guarantee its completeness or applicability to every patient.
No dentist-patient relationship; appointments and referrals
Viewing this Website or submitting an inquiry, appointment request, or referral does not, by itself, establish a dentist-patient relationship. Appointment requests require confirmation by our office. Referral submissions are reviewed and do not guarantee acceptance, treatment, or a particular appointment time. Clinical care, financial arrangements, cancellation policies, and informed consent are addressed separately.
Emergencies and electronic communications
The Website, online forms, email, and text messaging are not emergency services and may not be reviewed immediately. Call our office for urgent dental concerns. For a medical emergency, call 911 or seek immediate medical care.
Do not send detailed health information, dental images, Social Security numbers, or payment card information through general contact forms or ordinary email. Use designated secure services or call us for instructions. Ordinary email and text messages may carry privacy risks. Contact us to discuss alternative communication methods.
Privacy and HIPAA
Our Website Privacy Policy explains Website information practices. Our separate HIPAA Notice of Privacy Practices explains how we use and disclose protected health information and describes patient rights. Both are available through the Website or by contacting our office. Applicable law and the Notice of Privacy Practices govern protected health information. Acceptance of these Terms is not a HIPAA authorization or consent to treatment.
SMS messaging terms
If you opt in to our text messaging program, Southern Roots Endodontics may send appointment reminders, scheduling updates, and general two-way patient communications within the scope of your consent.
Message frequency varies. Message and data rates may apply.
Reply STOP to opt out or HELP for assistance. You may also contact our office to withdraw consent through another reasonable method.
We honor opt-out requests as required by applicable law. A one-time, nonpromotional confirmation of your opt-out may be sent where permitted.
Text messaging is optional and is not a condition of receiving dental treatment or purchasing services.
Marketing texts, if offered, require separate consent as required by law. Consent to patient communications does not automatically include promotional messages.
Provide only a telephone number you own or are authorized to use, and tell us if it changes. Our opt-in program is intended for adults age 18 or older, including parents or authorized guardians communicating about a minor's care.
Message delivery may be delayed or unavailable because of carrier or technical issues. Carriers are not liable for delayed or undelivered messages. Do not rely on texts for emergency communication.
Mobile opt-in information and consent data will not be shared with third parties or affiliates for marketing or promotional purposes. We may share this information with service providers solely to operate and support our messaging program, subject to applicable privacy requirements. Other legally permitted disclosures are described in our Privacy Policy and HIPAA Notice of Privacy Practices.
Appropriate use and submitted information
Patients, prospective patients, parents or authorized representatives, and referring dental professionals may use the Website for its intended purposes. Provide accurate information and submit another person's information only when authorized. Do not impersonate others, submit unlawful or infringing material, interfere with Website operation, introduce malicious software, or attempt unauthorized access to accounts, systems, or information.
We may restrict Website access to address misuse or security concerns. These Website access provisions do not determine termination of an existing clinical relationship, which is subject to applicable professional and legal obligations.
Intellectual property
Unless otherwise indicated, Website content and Southern Roots Endodontics branding are owned by or licensed to us and protected by applicable intellectual property laws. You may view, save, and print materials for personal, noncommercial use. Referring offices may use materials expressly supplied for patient education or referrals for those purposes. Other reproduction, modification, distribution, or commercial use requires permission unless otherwise permitted by law. Submission of patient records does not transfer ownership of those records to us under these Terms.
Accounts and third-party services
If you use an account or patient portal accessible through the Website, safeguard your login information and notify the relevant service and our office of suspected unauthorized access. Separate portal or service terms may apply.
Third-party links, maps, payment services, and other external features are provided for convenience. Their independent services may have their own terms and privacy policies. A link does not necessarily imply endorsement. Our own legal obligations concerning patient information remain applicable.
Website availability, warranties, and liability
We may change, suspend, or discontinue Website features. We do not guarantee continuous availability or freedom from errors. To the extent permitted by law, the Website and general informational content are provided “as is” and “as available,” without express or implied warranties, including merchantability, fitness for a particular purpose, and noninfringement.
To the extent permitted by law, Southern Roots Endodontics is not liable for indirect, incidental, special, or consequential damages resulting from Website use or inability to use it. These provisions concern Website use; they do not waive patient rights, excuse professional negligence, limit our HIPAA obligations, or exclude liability that cannot legally be excluded.
Tennessee law and general provisions
These Terms are governed by Tennessee law, except where federal law controls. Disputes concerning Website use are subject to the jurisdiction of courts of competent jurisdiction in Tennessee, unless applicable law requires otherwise. These Terms do not shorten any applicable statutory period for bringing a claim.
If a provision is unenforceable, the remaining provisions remain effective to the extent permitted by law. Failure to enforce a provision is not a waiver. We may revise these Terms by posting an updated version and effective date. Additional notice or consent will be provided where required by law.
